Job summary

AdviniaCare Seashore Pointe in Provincetown, MA seeks a compassionate Occupational Therapist to join our interdisciplinary team. You will help residents achieve their highest physical, cognitive, and emotional function by designing and implementing personalized treatment plans focused on independence, safety, and quality of life.

You will work closely with rehabilitation colleagues to assess needs, develop plans, and educate residents and families on exercises and techniques, with a commitment

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
