Job summary

AdviniaCare Northbridge (2) is seeking a compassionate Occupational Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

You will assess needs, adjust plans, educate residents and families, collaborate with the team, monitor progress, and maintain accurate records to support optimal outcomes.

Qualifications

• Licensed OT with current licensure.
• Experience in clinical rehabilitation settings preferred.
• Strong communication and documentation skills.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans addressing physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to coordinate care.
• Educate residents and families on exercises and techniques.
• Monitor and report on resident progress and outcomes.
