Job summary

AdviniaCare Summit Commons is seeking a compassionate Occupational Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety and quality of life for residents.

You will assess needs, educate residents and families on therapeutic exercises, and monitor progress while collaborating with other care professionals to ensure coordinated, patient‑centered care.

Qualifications

• Licensed Occupational Therapist (must be licensed in state of practice).

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
