The Phoenix Center is seeking a licensed Occupational Therapist (OT) to provide diagnostic and therapeutic services to evaluate, treat, document, and facilitate care for residents with impaired activities of daily living. The OT will participate in interdisciplinary team meetings and help develop ADL decline prevention policies with the Director of Nursing.
Candidates must have excellent communication and organizational skills, be able to collect data, define problems, and draw conclusions.

The purpose of this position is to provide diagnostic and therapeutic services to evaluate, treat, document, and/or facilitate care for residents with impaired activities of daily living due to disease process; participate in the interdisciplinary team meetings as required; assist the Director of Nursing Service to develop ADL decline prevention policies and procedures.
QUALIFICATIONS OF THE OCCUPATIONAL THERAPIST (OT):
