Job summary

Adaptive Home Health seeks a licensed Occupational Therapist to provide in-home OT visits across Texas. This full-time role emphasizes ADL/IADL training, safety enhancement, and independence for patients in their homes.

You will build care plans, train on adaptive equipment, and collaborate with nursing and the care team while documenting thoroughly in HCHB. Travel is daily across the assigned territory and scheduling is designed for field practicality.

Qualifications

• Active Texas OT license in good standing.
• Comfort working independently in the field.
• Strong communication and thorough documentation.
• Willingness to travel daily across assigned territory.
• Ability to carry/transport basic therapy equipment.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a plan of care focused on ADLs/IADLs, safety, and independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team as conditions change.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
