Job summary

Adaptive Home Health is seeking an Occupational Therapist to deliver in‑home evaluations and skilled visits. You will build a plan of care focused on ADLs/IADLs, safety, and independence while coordinating with nursing and the broader care team.

Expect to document thoroughly in HCHB and to work in a field-based role across assigned territories. The ideal candidate demonstrates strong clinical judgment, independence in the field, and clear communication.

Qualifications

• Active Texas OT license
• Strong clinical judgment and comfort working independently in the field
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow‑through on orders, frequencies, and patient goals
• Current driver’s license and comfort with daily travel across assigned territory
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Responsibilities

• Recommend and train on adaptive equipment and home modifications as needed.

• Schedule: Full-time Monday‑Friday + occasional weekends (as needed)

• Location: Field‑based

• Comp: 28PP

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last‑minute changes, and protect clinician time.

Requirements

Must‑haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfort escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow‑through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice‑to‑haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper experience in high‑signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

• Strong organization and time management
