Job summary

Adaptive Home Health in Lake Jackson is seeking a field-based Occupational Therapist to perform in-home evaluations and skilled visits focused on ADLs/IADLs, safety, and functional independence.

You will train on adaptive equipment, coordinate with nursing and the broader care team, and document promptly while leveraging scheduling tools to keep patient care as the priority.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.

• Schedule: Full-time Monday–Friday

• Location: Field-based (Lake Jackson Pod)

• Comp: $75.00 / point plus mileage reimbursement

Requirements

Must-haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Current driver’s license and comfort with daily travel across assigned territory

Nice-to-haves

• Prior home health experience

• Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

• Experience using charting systems / EMRs
