Job summary

Adaptive Home Health in Texas is seeking an Occupational Therapist (OT) for a field-based, full-time role. You will complete in-home evaluations and deliver skilled OT visits, focusing on ADLs/IADLs, safety, and independence.

You will collaborate with nurses and the broader care team, document in HCHB, and help patients adapt homes with equipment and modifications. A Texas OT license and strong clinical judgment are required.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Ability to escalate changes in condition to RN/MD and document rationale clearly.
• Strong communication and documentation skills.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and ability to travel daily across assigned territory.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment).
• Nice-to-haves: Home health experience, HCHB experience, neuro/CVA/fall risk experience.

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last‑minute changes, and protect clinician time.

What you will do

• Complete in-home OT evaluations and deliver skilled OT visits.

• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.

• Recommend and train on adaptive equipment and home modifications as needed.

• Coordinate with nursing and the broader care team when condition or needs change.

• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

Must‑haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfort escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow‑through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice‑to‑haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper experience in high‑signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
