Job summary

Adaptive Home Health is seeking an Occupational Therapist to perform in-home OT evaluations and visits across a field-based territory. The role is full-time, with Monday–Friday schedule and occasional weekend coverage as needed.

You will build a plan of care focused on ADLs/IADLs, safety, and independence, while coordinating with the care team and using Homecare Homebase (HCHB) for documentation. Travel across the assigned area is required.

Qualifications

  • Active Texas OT license in good standing.
  • Strong clinical judgment and comfort working independently in the field.
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
  • Strong communication and documentation.
  • Reliable follow-through on orders, frequencies, and patient goals.
  • Current driver’s license and comfort with daily travel across assigned territory.
  • Able to carry/transport basic therapy equipment as needed.
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).

Requirements

Must-haves

  • Active Texas OT license (in good standing)

  • Strong clinical judgment and comfort working independently in the field

Nice-to-haves

  • Prior home health experience

  • Homecare Homebase (HCHB) experience (or similar home health EMR)

  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

  • Strong organization and time management
