Job summary

Adaptive Home Health seeks a licensed Occupational Therapist for in-home visits in the Galveston area. You will evaluate patients, develop and implement plans of care emphasizing ADLs/IADLs, safety, and functional independence, and recommend adaptive equipment and home modifications as needed.

You will collaborate with nursing and care teams, document promptly, and travel daily within the territory. A strong field-oriented clinician with driving flexibility is essential.

Qualifications

• Active Texas OT license (in good standing).
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
• Reliable communication and thorough documentation.

Responsibilities

• Schedule: Full-time Monday–Friday
• Location: Field-based (Galveston pod)
• Comp: $75.00 / point plus mileage reimbursement

Requirements

Must-haves

• Active Texas OT license (in good standing)
• Strong clinical judgment and comfort working independently in the field
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow-through on orders, frequencies, and patient goals
• Current driver’s license and comfort with daily travel across assigned territory
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

• Prior home health experience
• Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
• Strong organization and time management
• Experience using charting systems / EMRs
