Job summary

Opis-Senior-Services-Grou is seeking a compassionate and licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team. The OT evaluates residents, develops individualized treatment plans, and provides occupational therapy services to promote independence, safety, and improved quality of life.

Responsibilites include evaluating residents, designing plans focused on ADLs and mobility, monitoring progress, and ensuring Medicare-compliant documentation.

Qualifications

• Master's or Doctorate degree in Occupational Therapy from an accredited program.
• Current OT license in good standing in the state.
• Knowledge of geriatric rehabilitation and Medicare regulations.
• Strong communication and documentation skills.

• Provide therapy focused on ADLs, functional mobility, strength, coordination, cognition, and safety.
• Monitor resident progress and adjust treatment plans as needed.

• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques and equipment.
• Maintain compliance with federal, state, and facility regulations.
