Job summary

Adaptive Home Health is seeking an Occupational Therapist to provide in-home OT evaluations and skilled visits across Texas. You will develop care plans focused on ADLs/IADLs, safety, fall prevention, and independence, while recommending adaptive equipment and home modifications to support patient goals.

Ideal candidates will have Texas OT licensure, strong clinical judgment, and the ability to work independently in the field with reliable travel.

Qualifications

• Active Texas OT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Ability to escalate changes to RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on orders and patient goals.
• Current driver’s license and daily travel across assigned territory.
• Able to carry/transport basic therapy equipment.
• Competency in ADL/IADL training, home safety, adaptive equipment, caregiver education.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits.

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last‑minute changes, and protect clinician time.

Requirements

Must-haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfort escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow‑through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper experience in high‑signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

• Strong organization and time management
