Job summary

Summit Home Care and Hospice seeks an experienced Occupational Therapist (OT) to join our field team providing in-home care in Grove City, OH and surrounding areas. This role involves on-the-road patient visits with occasional training at the local office and remote work as appropriate.

The ideal candidate will hold an accredited OT degree, be licensed in Ohio, and have strong communication and independent work skills.

Qualifications

• Graduate of an accredited occupational therapy curriculum.
• Registered with the NBCOT (National Registration Examination).
• Active license to practice OT in Ohio.
• 2 years of clinical OT experience preferred, especially in home health or community settings.

Responsibilities

• Evaluate patients' functional status and home environment.
• Develop treatment plans and goals to improve function.
• Perform work capacity evaluations and refer to vocational training if needed.
• Instruct and train patients in daily activities and use of adaptive equipment.
• Coordinate care with physicians and other health professionals.
• Supervise assistants and ensure proper documentation.
• Participate in discharge planning and patient family education.
