Job summary

Human Touch Home Health seeks a PRN Occupational Therapist to deliver home-based care in Fredericksburg and surrounding communities. You’ll perform home visits, evaluate function, and build treatment plans around patient availability.

This role offers a sign-on bonus, flexible scheduling, mileage reimbursement, and one-on-one patient care under supportive leadership. Virginia-licensed OTs with driving means can join immediately.

Qualifications

• Active Virginia OT license required.
• Graduate of an accredited occupational therapy program.
• Valid driver’s license and reliable transportation.
• WellSky experience a plus.
• Strong clinical and communication skills.

Responsibilities

• Complete OT evaluations in patients' homes.
• Develop individualized treatment plans.
• Help patients regain independence in ADLs.
• Provide interventions to improve strength and mobility.
• Assess home safety and recommend equipment.
• Educate patients and caregivers; monitor progress.
• Coordinate care with the care team; document in WellSky.

SIGN-ON BONUS AVAILABLE! Flexible Schedule. Local Territory. One-on-One Patient Care. Human Touch Home Health is hiring a PRN Occupational Therapist (OT) to serve patients throughout Fredericksburg and surrounding communities — and we're offering a SIGN-ON BONUS!

Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and surrounding areas.

You do NOT have to cover the entire territory. Choose the areas that work best for you and build your caseload around your availability.

What You'll Do

• Complete OT evaluations in patients' homes
• Develop individualized treatment plans
• Help patients regain independence with activities of daily living
• Provide therapeutic interventions to improve strength, coordination, mobility, and function
• Assess home safety and recommend adaptive equipment or modifications
• Educate patients and caregivers
• Monitor patient progress and adjust treatment plans
• Coordinate care with nurses, physicians, therapists, and other team members
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Occupational Therapist License
• Graduate of an accredited Occupational Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in a home health setting
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
