Job summary

Lilac Manor Nursing and Rehabilitation Center is seeking a dynamic Occupational Therapist for our skilled nursing facility. You will develop and implement the OT plan of care for residents and short-term patients, and participate in routine screenings.

You will document progress, communicate with an interdisciplinary team, provide training to patients and families, and help maintain a safe rehab area. NYS OT license is required; geriatric care preferred.

Qualifications

• Must be licensed as an Occupational Therapist in New York State.
• One year of physical therapy experience in a geriatric setting preferred.

Responsibilities

• Develop and implement the Occupational Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screenings for the need for Occupational Therapy.
• Communicate progress and any barriers toward OT goals with interdisciplinary team members.
• Complete timely OT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each OT patient’s specific plan of care and recommendations for follow up.
• Perform additional duties as required.
