Lilac Manor Nursing and Rehabilitation Center is seeking a dynamic Occupational Therapist to join our skilled nursing facility team. You will develop and implement OT plans of care, conduct screenings, and collaborate with an interdisciplinary team to improve residents' lives.

Responsibilities

• Develop and implement OT plan of care for residents and short-term patients.
• Assist with patient screenings for OT needs.
• Communicate progress and barriers toward OT goals with the interdisciplinary team.
• Complete timely OT documentation in accordance with regulatory and payor requirements.
• Maintain rehab gym equipment and treatment area with infection prevention practices.
• Provide patient, family, and caregiver training on each plan of care and follow-up recommendations.
• Perform additional duties as needed.

Lilac Manor Nursing and Rehabilitation Center is looking for a dynamic Occupational Therapist for one of our skilled nursing facilities! DUTIES AND RESPONSIBILITIES:

• Develop and implement the Occupational Therapy plan of care for facility residents and short-term patients.

• Assist as needed with patient screenings for the need for Occupational Therapy.

• Communicate progress and any barriers toward OT goals with interdisciplinary team members.

• Complete timely OT documentation in compliance with regulatory, licensing, and payor requirements.

• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

• Provide and document patient, family, and caregiver training regarding each OT patient’s specific plan of care and recommendations for follow up.

• Perform additional duties as required.

Education and Experience Requirements:
