Job summary

The Grand Rehabilitation and Nursing at River Valley in Poughkeepsie, NY is seeking a compassionate and skilled Occupational Therapist to join our rehabilitation team. You will assess residents, develop treatment plans, and help them regain independence.

The ideal candidate has graduated from an accredited OT program, holds a NY license, and values compassionate patient care. Experience is a plus, and we offer competitive compensation and a comprehensive benefits package.

Responsibilities

• Evaluate and develop treatment plans for residents based on medical diagnosis
• Implement treatment plans to improve residents' functional independence
• Assess and document progress of residents in detailed re-certifications, progress reports, and daily notes
• Maintain documentation of Resident care services by auditing Resident & Department records
• Supervise occupational therapy assistants and participate in care plan meetings and documentation
• Plan, develop, and organize OT-related reports and data tracking as directed by the Rehab Director
• Assist in departmental projects as needed
