Job summary

Alternate Solutions Health Network is seeking an Occupational Therapist to provide home-based OT services for patients in the North Kansas City area. You will collaborate with an interdisciplinary team to deliver compassionate care, educate patients and families, and develop an individualized plan of care under physician direction.

The role requires an OT license in the state, at least one year of acute care OT experience, and a valid driver’s license.

Qualifications

• OT license in the state of employment is required.
• Minimum one year of OT experience in an acute care setting.
• Home care experience is preferred.
• Valid driver's license and auto insurance in your name as a driver.

Responsibilities

• Provide skilled occupational therapy as ordered by the attending physician.
• Educate patients and families and ensure patient safety.
• Complete accurate documentation and comply with regulations.
• Develop individualized OT plan of care and adjust as needs change.
• Evaluate patient's functioning level and progress.
• Use home exercise programs to improve ROM, strength, and coordination.
• Educate and advocate for patients and families about disease/injury management.
• Supervise COTA and communicate supervisory visit results.
• Follow agency policies, Medicare/Federal guidelines and maintain compliance.

HOW YOU'LL WORK

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient. You are accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.

MAJOR AREAS OF RESPONSIBILITY

• Plan of Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Develop an individualized OT plan of care and make adjustments as needs change.
• Evaluation: Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
• Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.
• Minimize Patient Risk: Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
• Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
• Supervision: Oversee and supervise total care of patient provided by COTA.
• Communication/Collaboration: Communicate with the COTA on supervisory visit results.
• Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

REQUIREMENTS

• Occupational Therapist with a current license in the state of employment.
• Minimum one-year experience as a OT in an acute care setting.
• Home care experience preferred.

• Capable of all physical demands.
