Job summary

Saunders Nursing and Rehabilitation Center is seeking a compassionate PRN Occupational Therapist to join our dynamic rehabilitation team. You will evaluate residents, create individualized treatment plans, and deliver skilled OT interventions to promote independence in daily activities and mobility.

Work collaboratively with therapists, nursing staff, physicians, residents, and families. Maintain accurate documentation, ensure safety and regulatory compliance, and participate in care conferences

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in Pennsylvania in good standing.
• CPR certification preferred.
• Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred.
• Experience with Net Health documentation software preferred.
• Experience with PointClickCare (PCC) preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans based on clinical findings, functional abilities, and resident goals.
• Provide skilled occupational therapy interventions focused on improving independence with activities of daily living, mobility, coordination, strength, and overall functional performance.
• Implement evidence-based treatment strategies to support residents in achieving their highest level of independence.
• Monitor resident progress and adjust treatment plans as needed to support positive outcomes.
• Accurately document evaluations, treatments, progress notes, and other required clinical information in a timely manner.
• Collaborate with interdisciplinary team members to support resident care goals and discharge planning.
• Educate residents, families, and caregivers regarding treatment plans, safety techniques, adaptive strategies, and functional improvement goals.
• Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines.
• Participate in care conferences and team meetings as appropriate.
• Ensure a safe and therapeutic treatment environment for all residents.
