Job summary

Madisonville Care Center in Texas seeks a compassionate PRN Occupational Therapist to improve residents' independence and quality of life. You will assess, plan, and implement OT services focused on ADLs/IADLs, while collaborating with an interdisciplinary team.

Deliver treatments per individualized plans, modify interventions as needed, and provide education to patients, families, and caregivers. Thorough documentation and regulatory compliance are required.

Qualifications

• Active and valid Texas OT license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Current knowledge of OT treatment practices.
• Experience with Net Health documentation preferred.
• Strong communication and collaboration skills.
• Ability to manage patients with varying needs.

Responsibilities

• Consult with patients to evaluate physical condition and impact on daily activities.
• Assess test results and interpret evaluations.
• Develop individualized OT treatment plans for ADLs/IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training.
• Deliver treatments per the plan of care.
• Monitor response and adjust interventions with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with team to ensure high-quality care.
• Complete timely documentation in line with policies and regulations.
• Attend required meetings as designated by Director of Rehab.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents' independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life

Assess and interpret patient evaluations and test results

Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)

Assess patients' home and living environments and recommend modifications based on their needs and capabilities

Recommend adaptive equipment and provide training to support independence and safety

Ensure all treatments are delivered in accordance with the established plan of care

Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team

Provide clinical education and support to patients, family members, and caregivers

Collaborate with interdisciplinary team members to ensure high-quality patient care

Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

Comply with all applicable rules, regulations, and procedures

Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Skilled Nursing Facility (SNF) Or Long-term Care Experience Preferred
• Current knowledge of occupational therapy treatment practices
• Experience With Net Health Electronic Documentation System Preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
