Job summary

Oasis Therapy Services in Madison, Wisconsin, seeks an Occupational Therapist (OT) to provide one-on-one rehabilitation in an assisted living setting. PRN position with Net Health as the EMR.

The role focuses on ADLs, mobility, safety, and independence, requiring an active WI OT license, graduation from an OT program, CPR certification, and strong communication skills. Experience in geriatric care is preferred.

Qualifications

• Active and current Wisconsin Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.
