Job summary

Evolve Therapy Services in Franklin, MN is seeking an Occupational Therapist for an in-house Skilled Nursing Facility to deliver comprehensive OT services.

You will screen residents, develop and adjust treatment plans, supervise a COTA, document care, and coordinate with the care team to ensure quality resident outcomes and accurate billing.

Qualifications

• Must hold a Degree in Occupational Therapy and an active OT license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screenings to determine best interventions for residents.
• Evaluate residents to inform treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and the care team.
• Establish therapy schedules in coordination with team members.
• Deliver OT services per plan and document and bill accordingly.
• Perform other duties to effectively treat patients as required.
• Oversee COTA and ensure compliant documentation and reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!) , Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
