Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Franklin, MN. The role centers on planning, directing OT services to ensure resident care of the highest quality.

You will conduct screenings, evaluate residents, and develop treatment plans with achievable goals, while communicating findings to residents, families, and the interdisciplinary team. Active licensure and a degree are required, with flexible PT/FT/PRN options and benefits.

Responsibilities

• Screen residents to identify best intervention.
• Evaluate residents to set short- and long-term goals.

• Supervise COTA and ensure compliant documentation and reimbursement.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Full Time ($5,000 sign on bonus and offering tuition reimbursement!), Part Time, or PRN positions available

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
