Job summary

Stonecreek Health and Rehabilitation in Paducah, KY is seeking an Occupational Therapist to join a warm, interdisciplinary team. You will provide OT services, screen and evaluate patients, develop and monitor treatment plans, discharge planning, and supervise assistants, all within regulatory guidelines.

You'll collaborate with therapy and facility staff to improve patient independence, safety, and quality of life, maintain licensure, and stay current with continuing education.

Qualifications

• Graduate of an accredited occupational therapy program.
• Valid state Occupational Therapist license, where applicable.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Reports to the Director of Rehabilitation.
• Provide occupational therapy services, including screening, evaluation, and treatment.
• Comply with state and federal guidelines related to documentation and billing.
• Establish and maintain effective, patient-centered interactions with patients and staff.
• Refer patients for treatment to assistants and provide supervision.
• Directly supervise occupational therapy assistants and rehab technicians.
• Participate in utilization review, care conferences, and interdisciplinary team activities.
• Develop and enhance OT programs to support quality outcomes.
• Ensure compliance with safety, infection control, and patient rights policies.
• Maintain clinical expertise through continuing education and licensure.
