Job summary

Evolve Therapy Services, LLC in Key West, FL is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility.

You will assess residents, develop and implement treatment plans, communicate findings to families and the interdisciplinary team, schedule therapies, document and bill for services, and collaborate to maintain high quality resident care.

This role offers flexibility with Full Time, Part Time, or PRN options and requires compassionate, patient-centered care.

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
