Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Tampa, FL. The role is part-time or PRN, with duties to plan, organize and direct OT services in the facility, ensuring compliance with standards and resident care quality.

The successful candidate will screen, evaluate, and develop treatment plans, communicate with residents and families, schedule therapy in coordination with the team, and oversee COTA supervision while ensuring proper

• OT degree and active license required.
• Ongoing continuing education to maintain licensure.
• Licensure in practicing state(s) required.

Responsabilidades

• Screen residents to determine best OT intervention for each patient.
• Evaluate residents and develop short- and long-term goals.
• Communicate findings to residents, families, and care team.
• Set up individual therapy schedules with team cooperation.
• Document treatments and support billing according to plan.
• Supervise COTA and ensure clinical excellence and proper reimbursement.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Part-Time or PRN

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
