Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist for an in-house Skilled Nursing Facility in Key West, FL. The role supports a patient-centered OT program across full-time, part-time, or PRN schedules.

Responsibilities include screening, evaluation, therapy planning, scheduling, documentation, and collaboration with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Job Position: Occupational Therapist for an in-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:
