Job summary

Evolve Therapy Services in Key West, FL, is seeking a dedicated Occupational Therapist to provide in-house therapy at a Skilled Nursing Facility. You will plan, implement, and supervise OT services, coordinating with a multidisciplinary team to deliver resident-centered care.

This full-time role offers salaried or flexible scheduling (4x10s available), competitive pay, and benefits including medical, dental, and vision coverage.

Qualifications

• OT degree required.
• Active OT license in practicing state(s) required.
• Commitment to ongoing continuing education.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.
