Job summary

Evolve Therapy Services in Key West, FL, is seeking a licensed Occupational Therapist to provide in-house therapy services at a Skilled Nursing Facility partner. This full-time, part-time, or PRN role offers flexible scheduling including 4x10s.

You will assess residents, develop and implement individualized treatment plans, coordinate with an interdisciplinary team, document and bill accurately, and oversee COTA as needed to ensure high quality care.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents to inform treatment plans and set goals.
• Interpret and share findings with residents, families, and the team.
• Establish individual therapy schedules with the team.
• Perform therapy and ensure proper documentation and billing.
• Oversee COTA and ensure compliant documentation and care.
