Job summary

Vintage Faire Nursing & Rehab in Modesto, CA is seeking a full-time Occupational Therapist to join our in-house therapy team. The role offers a $5,000 sign-on bonus and an hourly pay range of $68–$72 based on experience.

We provide a supportive environment focused on geriatric rehabilitation and program development. Qualified candidates are licensed Occupational Therapists, with opportunities for mentorship, leadership development, and ongoing clinical education within a dynamic team.

Qualifications

• Licensed Occupational Therapist credential required.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive or ADL impairments and develop care plans.
• Use standardized assessments to guide interventions.
• Provide rehabilitative, skilled treatment per clinical standards.
• Administer ADL retraining, therapeutic exercises, modalities and assistive devices.
• Track progress and adjust treatment plans in the medical record.
• Provide discharge summaries per regulatory requirements.
