Job summary

Embassy of Hearthside in State College, PA is seeking a full-time Occupational Therapist (OT) to join our in-house rehabilitation team. The role focuses on personalized, resident-centered therapy and developing treatment plans to improve independence, safety, and daily functioning.

Qualifications

• Active Occupational Therapy license in PA, in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.
• Familiarity with Net Health EDS is preferred.

Responsibilities

• Evaluate residents’ functional abilities and their impact on daily activities.
• Develop and implement individualized OT treatment plans.
• Provide skilled therapy to improve independence in ADLs/IADLs.
• Recommend adaptive equipment and home modifications as appropriate.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, caregivers, and clinical staff.
• Collaborate with interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet professional, legal, and regulatory standards.
