Job summary

Hearthside Nursing Home in State College, PA is seeking a full-time Occupational Therapist (OT) to join our in-house rehabilitation team. New graduates are welcome to apply.

You will provide resident-centered therapy, evaluate function, and develop individualized plans to improve daily activities and independence. Join a collaborative rehab program with opportunities to recommend adaptive equipment, monitor progress, and educate residents and families.

Qualifications

• Active OT license in PA in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.
• Familiarity with Net Health documentation system preferred.

Responsibilities

• Evaluate residents’ functional abilities and impact on daily activities.
• Develop and implement individualized OT treatment plans.
• Provide skilled therapy focused on ADLs and IADLs.
• Recommend adaptive equipment and home modifications as appropriate.
• Monitor progress and adjust OT plans as needed.
• Educate residents, families, caregivers, and staff.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely documentation.
• Ensure services meet professional, legal, and regulatory standards.

Key Responsibilities

• Evaluate residents’ functional abilities and their impact on daily activities
• Develop and implement individualized occupational therapy treatment plans
• Provide skilled therapy services focused on independence in activities of daily living and instrumental activities of daily living
• Recommend adaptive equipment and home modifications as appropriate
• Monitor resident progress and adjust OT treatment plans as needed
• Educate residents, families, caregivers, and clinical staff
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Ensure services meet all professional, legal, and regulatory standards

Qualifications

• Active Occupational Therapy license in PA, in good standing

• Knowledge of the Net Health Electronic Documentation System preferred
