Community Physical is seeking an Occupational Therapist (OT) to provide comprehensive therapy services across Skilled Nursing, memory care, outpatient, and home care settings in the greater Chicagoland area. The position operates at Alden Terrace in McHenry, IL, with either full-time or part-time hours.

Candidates must hold a current Illinois OT license. SNF experience is preferred; Net Health documentation experience is a plus.

Qualifications
