Enhance Rehabilitation in Illinois is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care to promote independence and safety.

In this role you will collaborate with facility staff, document progress, educate patients and families, and supervise Occupational Therapy Assistants to ensure consistent, high-quality care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
