Job summary

Enhance Rehabilitation in Illinois is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, follow an individualized plan of care, and collaborate with facility staff to ensure high-quality outcomes.

The role involves developing plans of care, setting measurable goals, educating patients and families, documenting progress, and supervising Occupational Therapy Assistants as needed.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
