Enhance Rehabilitation in Illinois is seeking qualified Occupational Therapist candidates to join our dynamic team. You will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will collaborate with facility staff, develop plans of care with measurable goals, educate patients and families about progress, re-evaluate progress, document notes, and supervise Occupational Therapy Assistants.

• Collaborates with facility staff to ensure high-quality patient care.
• Documents evaluations, progress notes and discharges in a timely and accurate manner.
• Supervises Occupational Therapy Assistants.

Responsibilities

• Providing excellent care for our patients.
• Providing excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for you patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
