Pride Health is hiring an Occupational Therapist for a home health assignment in Epping, NH. This 13-week local contract offers a schedule of Monday to Friday, 8:00 AM to 4:30 PM, with 32-40 hours guaranteed weekly and mileage reimbursement.

On W2, pay rate ranges from $46 to $53 per hour depending on experience. Responsibilities include evaluating patient needs, developing therapy plans, coordinating with the care team, and documenting progress.

Qualifications

• Current Occupational Therapy license in NH.
• HCHB experience preferred.
• Current CPR certification.
• Valid driver’s license with auto insurance and reliable transportation.

Responsibilities

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Provide OT services within scope of practice per plan of care and in coordination with health care team.
• Evaluate functional status and CMS-required information for documentation.
• Consult with physician in development of therapy plan of care.
• Observe, record, and report patient response to treatment and changes in condition.
• Conduct patient assessments using functional ability tests.

Schedule: Mon - Fri (5 days/wk.) 8 AM to 4:30 PM (32-40 hrs. weekly guaranteed)

Mileage: Reimbursement provided (from first patient to last patient)

On Call/ Weekend: No

Job Responsibilities

• The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
• Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs.
• Consults with the physician in the development of the therapy plan of care.
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Qualifications

• Current Occupational Therapy license in the state of practice.
• Home Health & Homecare Homebase (HCHB) experience highly preferred.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
