Resource 1 Homecare Staffing is seeking a Home Health Occupational Therapist to deliver one-on-one OT services in patients' homes across Concord, Tilton and Manchester, NH. You will evaluate patients, develop personalized treatment plans, and help individuals regain independence in daily activities while coordinating with physicians and the interdisciplinary care team.

This full-time role offers a Monday–Friday schedule, 8:00 AM–5:00 PM, with competitive pay and strong benefits, including PTO,

Qualifications

• Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited program.
• Current Occupational Therapist license in New Hampshire.
• Strong communication and interpersonal skills.
• Excellent organizational and time management abilities.
• Comfortable using electronic medical records (EMR).
• Home health experience preferred but not required.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans.
• Help patients improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Evaluate upper extremity strength, coordination, cognition, perception, and functional mobility.
• Recommend adaptive equipment, assistive devices, and home modifications to improve safety and independence.
• Educate patients and caregivers on therapeutic techniques, fall prevention, energy conservation, and home safety.
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other members of the care team.
• Complete timely and accurate documentation using KanTime EMR.
• Participate in interdisciplinary case conferences and care planning.

Position Summary
