Sanctuary Personnel Ltd. is seeking a Locum Occupational Therapist for the Home First Team in Wareham, Dorset. This ongoing full-time role offers hybrid working and the chance to support hospital discharge planning and reablement in the community.

You will deliver OT assessments, collaborate with health and social care partners, and manage a caseload across Dorset. Applicants should have HCPC registration, 3+ years UK OT experience, and be prepared for travel.

Qualifications

• Degree in Occupational Therapy with HCPC registration.
• Minimum 3 years post-qualified UK Occupational Therapy experience.
• Experience in hospital discharge pathways and reablement services.
• Ability to travel Dorset with a car and work collaboratively with teams.
• Enhanced Adult DBS and Right to Work in the UK.
• Experience within Home First services and community OT.

Responsibilities

• Support hospital discharges and discharge planning for seamless transitions.
• Deliver reablement-focused Occupational Therapy interventions to promote recovery and independence.
• Complete comprehensive OT assessments in community settings.
• Collaborate with health and social care partners and multidisciplinary teams.
• Manage a community-based caseload and perform home visits across Dorset.
• Maintain accurate records and documentation of cases.
