Job summary

EvergreenHealth, a leading home health provider in the Pacific Northwest, seeks an Occupational Therapist to assess, plan, implement, and evaluate skilled home-based care. You will work with an interdisciplinary team to ensure quality outcomes and patient safety in community settings.

Responsibilities include developing and updating the plan of care, educating patients and families, and coordinating with physicians and case managers.

Qualifications

• Current OT licensure to practice Occupational Therapy in Washington State.
• Recent clinical experience in a medical/surgical unit, rehab, or home health setting.
• Washington driver’s license and proof of insurability.
• Current Healthcare Provider BLS certification by date of hire.
• Reliable transportation.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Educate patients and caregivers to promote maximal functional status in the home.
• Regularly evaluate and re-evaluate patient status.
• Develop and revise the plan of care (POC) with the interdisciplinary team.
• Communicate with other disciplines involved in the case.
• Report signs/symptoms signaling a change to the physician and case manager as needed.
• Reassess the patient within 48 hours after a change or discharge.
