Job summary

UnitedHealth Group's Caretenders Home Health, part of LHC Group within Optum, seeks an Occupational Therapist to assess and evaluate patients' functional status and daily living activities in home health settings in Columbus, OH.

You will collaborate with a care team, develop therapy plans, conduct home visits, document progress, and ensure safe, effective care for diverse patients. A state license is required and a valid driver's license with reliable transportation is preferred.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Assesses and evaluates patient care needs related to functional status and activities of daily living.
• Performs home assessments and selects adaptive equipment as needed.
• Coordinates with the health care team and physician on the therapy plan of care.
• Documents patient progress and reports changes to the nurse supervisor.
• Operates safely in home settings respecting patient diversity.
