Job summary

UnitedHealth Group is seeking an Occupational Therapist for Home Health in Canton, OH. You will assess and address patients' functional status, ADLs, and equipment needs within the plan of care, coordinating care with physicians and the health team.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver's license and access to a dependable vehicle or public transportation.
• Ability to function respectfully in diverse home settings regardless of client background.

Responsibilities

• Provides OT services within state scope of practice and plan of care, coordinating with the health care team.
• Evaluates functional status and CMS-required documentation; communicates with physician on therapy plan of care.
• Observes and reports patient responses to treatment and changes in condition to nurse supervisor and physician.
• Conducts patient assessments of function using diagnostic and prognostic tests.
