Job summary

Commonwealth Home Health, part of the LHC Group within the Optum family, is hiring an Occupational Therapist for home health. You will assess patient needs, develop therapy plans, and coordinate with the care team to support daily living activities in patients' homes.

Responsibilities include evaluating function, documenting CMS-required information, and communicating with physicians. Licensure, reliable transportation, and CPR certification are preferred.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred) or ability to obtain within 90 days.

Responsibilities

• Provides service within the scope of practice defined by state laws and plan of care.
• Evaluates patient's functional status and coordinates with physician on therapy plan.
• Observes and reports on patient response to treatment and changes in condition.
• Conducts patient assessments evaluating level of function using standardized tests.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
