Elite Home Health, part of LHC Group and within the Optum family, is seeking a PRN Occupational Therapist in Home Health. This role focuses on assessing and evaluating patient care needs related to functional status, activities of daily living, and adaptive equipment, with coordination with the health care team.

You will perform home health OT services, document progress, and contribute to care planning in diverse home environments.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to reliable transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Ability to manage multiple tasks and work independently.

Responsabilités

• Provides service within the scope of practice as defined by state laws and plan of care.
• Evaluates the patient’s functional status and needs; consults with physician for therapy plan of care.
• Observes, records, and reports patient progress to nurse supervisor and physician.
• Conducts patient assessments evaluating function using appropriate tests.
• Function in diverse home settings regardless of client background.
