Pathways Healthcare is seeking an Occupational Therapist to provide patient-centered care in home health and hospice. The role focuses on comprehensive assessments, individualized treatment plans, and caregiver education across Cambridge, Revere and surrounding towns.

The position offers a Monday–Friday schedule (8:30–5:00pm) with full-time employment, a competitive salary range, and a full benefits package including PTO, holidays, and health coverage.

• Flexibility to work in a home-based setting and coordinate with families.

Responsabilités

• Conduct comprehensive patient assessments in home environments.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized OT treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Refer patients to additional services as needed and coordinate discharge planning.

Job Description:

• Role: Occupational Therapist
• Salary: $80,000-$105,000 per year
• Main Office location: Westwood, MA
• Service Area: Cambridge, Revere and Surrounding Towns
• Schedule: Monday - Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Educate families and caregivers on treatment plans.

• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.
