Job summary

Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, offers a meaningful Occupational Therapy role. You will assess patients at home, develop treatment plans, and train patients in daily activities and adaptive equipment to support recovery with dignity and purpose.

You will supervise assistants, coordinate discharge planning, and educate families, serving Springfield and surrounding areas in Virginia with per-visit compensation and mileage

Qualifications

• Hold a degree from an occupational therapy curriculum accredited jointly by the AMA/AOTA and be registered with the NBCOT.
• Minimum of two years of clinical OT experience preferred.
• Excellent verbal and written communication with strong organizational skills.

Responsibilities

• Conduct comprehensive patient assessments including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.

Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.
