Job description

BAYADA Home Health Care

is currently seeking full-time, part-time, and per diem Occupational Therapists to join our team in our Harrisburg Visits Office.

This office services adults and geriatric clients on a per visit basis in Halifax, Dauphin, Millersburg, Lykens, Elizabethville, and surrounding areas.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care to remain at home.

Occupational Therapist, OT Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, communication skills, and ADL and IADL performance.
3. Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
4. Evaluate the home environment for hazards or barriers to independent living.
5. Identify equipment needs and environmental adaptations to enhance client outcomes.
6. Continuously assess and revise the occupational therapy care plan, collaborating with nursing in the multidisciplinary care plan as appropriate.
7. Educate and instruct patients, family members, or other representatives in rehabilitative care to promote health, safety, and independence.
8. Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Occupational Therapist, OT Qualifications:

1. Current PA Occupational Therapist license.
2. Prior home care experience preferred but not required.
3. At least one year of recent work experience as an Occupational Therapist.
4. Graduation from an accredited occupational therapy program as indicated by transcript or diploma.
5. Ability to work independently and manage time effectively.
6. Strong interpersonal, organizational, and problem-solving skills.
7. Solid computer skills; experience with electronic medical records (EMR) preferred.
