Job summary

Pathways Healthcare is seeking a licensed Occupational Therapist to join our in-home rehab team. You will assess patients in their homes, develop personalized treatment plans, and guide patients through activities of daily living and adaptive equipment use.

Qualifications

• Hold an OT degree from an accredited curriculum and be NBCOT registered.
• Minimum two years of clinical OT experience preferred.
• Excellent verbal and written communication and strong organizational skills.

Responsibilities

• Conduct comprehensive patient assessments and evaluate home environment needs.
• Develop and implement individualized therapy plans with achievable goals.
• Train patients in activities of daily living, prosthetics, and adaptive equipment use.
• Educate families and caregivers and supervise assistants and aides.

Responsibilities:

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.
• Salary Range: $80,000 - $90,000
