Job summary

Pathways Healthcare is seeking an Occupational Therapist to provide home health services in Beverly and surrounding towns. You will assess patients in their homes, develop individualized plans, and support patients’ daily living activities with adaptive equipment and training.

The role requires an OT degree from AMA/AOTA accredited program and active registration. A valid driver’s license is required; per diem applicants may also apply.

Qualifications

• OT degree accredited by AMA and AOTA.
• Registered with the National Registration Exam (AOTA).
• Minimum two years of OT experience preferred.
• Valid driver’s license required.

Responsibilities

• Conduct comprehensive patient assessments in home settings.
• Develop and implement individualized OT treatment plans.
• Train patients in daily activities, prosthetics, and adaptive devices.
• Educate families and caregivers on treatment plans.

Job Description:

• Role: Occupational Therapist - Home Health
• Service Area: Beverly, Danvers, Lynn, Lynnfield, Malden, Middleton, North Reading, Peabody, Reading, Salem, Saugus, Swampscott, and Wakefield areas.
• Schedule: Monday-Friday (8:30-5:00pm)
• Job Type: Full Time; Per Diem applicants are welcome to apply.

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.
