Pathways Healthcare is seeking an Occupational Therapist for home health in the Concord, MA area. You will assess patients in their homes within the service area and develop individualized OT plans to restore function and improve quality of life.

Join a patient-centered team that values clinical judgment, documentation efficiency, and collaborative care, with a schedule Mon-Fri, 8:30–5:00, and a strong benefits package.

Qualifications

• OT degree from an accredited program and NBCOT registration.
• Minimum two years' OT experience preferred.
• Excellent verbal and written communication; strong organizational skills.
• Valid driver's license and own vehicle.

Responsabilités

• Assess patients' functional status and home environment.
• Develop and implement individualized OT treatment plans.
• Train patients in activities of daily living and assistive devices.
• Supervise OT assistants and aides as needed.
• Document patient progress and coordinate with care teams.
• Educate families and coordinate discharge planning.

Job Description:

• Role: Occupational Therapist - Home Health
• Main Office Location: Concord, MA
• Service Area: Devens, Shirley, Ayer, Dunstable, Groton, Pepperell, Tyngsboro, Townsend, Fitchburg, Leominster, Lunenburg areas.
• Schedule: Monday-Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• Hold a degree from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. Additionally, be registered with the National Registration Examination of the American Occupational Therapy Association.
• A minimum of two years of clinical experience as an Occupational Therapist is preferred.
• Demonstrate excellent verbal and written communication skills along with strong organizational abilities to effectively interact with patients and the healthcare team.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Conduct comprehensive patient assessments, including functional status, home environment, and equipment needs.
• Evaluate work capacity and provide vocational training referrals when necessary.
• Develop and implement individualized treatment plans with achievable goals.
• Train patients in daily activities, prosthetics, orthotics, and adaptive equipment use.
• Maintain accurate records and report patient condition changes promptly.
• Educate families and caregivers on treatment plans.
• Supervise occupational therapy assistants and home health aides.
• Participate in training, meetings, and case reviews.
• Refer patients to additional services as needed and coordinate discharge planning.
